California pharmacies will soon be required to report medication errors through the California Medication Error Reporting (CAMER) system. Starting September 1, 2025, all medication errors must be submitted through CAMER, which is operated in partnership with the Institute for Safe Medication Practices (ISMP).

To comply with this upcoming requirement, pharmacies and professionals must first register with ISMP’s reporting platform. You can begin the registration process and learn more by visiting: https://mederrors.ismp.org/.
